QUESTION........ WHEN ANOTHER FD GOES INTO ANOTHER DIST. THE USUALLY NOTIFY THAT DIST THAT THEY ARE WORKING WITHIN IT. (USUALLY) WELL N.S. EMS IS TAKING AIDED CASES IN DISTRICTS AND NOT NOTIFYING THEM THEY ARE ENTERING AND TAKING CALLS FROM FD`S OR VAC`S. APPARENTLY DOCTORS ARE TELLING PT`S TO CALL N.S. EMS AND THEY WILL TAKE THEM WHERE EVER THEY WISH...............
First of all there is NOTHING wrong with that. The only problem is that the local doctors in your town think you guys are awful, so they are calling someone else. Maybe you should stop complaining and improve your crappy service.

Read Article 30 of the NYS Public Health law reguarding response to a call in any district in Suffolk County by a private ambulance service
SECTION 3017. EMERGENCY MEDICAL SERVICE, SUFFOLK COUNTY. [Expires and Repealed January 1, 2003]
1. No ambulance service or advanced life support first response service shall respond to any call or request for emergency medical services within a town, village or fire district in Suffolk County that currently provides ambulance service or advanced life support services first response service, if the municipality has designated one or more ambulance services or advanced life support first response services to respond to such calls unless:
(a) The service is North Shore LIJ, because we, up here in Albany think North Shore LIJ rule too.
(b) the response is in accordance with a mutual aid plan approved by the appropriate regional emergency medical services council;
(c) the response is to a verbal mutual aid request from a designated service, like you saying "Help North Shore LIJ we cant get out on any of our call because we suck"
(d) the service was specifically requested to respond by the patient or someone acting on behalf of that patient
(e) the response site is a hospital licensed under Article 28 of this chapter for a transfer to another such facility.

Private ambulances companies are now allowed to provide mutual aid EMS also. Any fire department or ambulance corps can hire a private to cover standbys, or provide 911 service in their absence. StonyBrook is doing it now for several areas. North Shore is already operating in Islip Town..all the corps agreed to ask for the service. Most areas are going to need backup sooner or later, by hiring people or utilizing other services. Everyone should wake up and realize that this is a fact of the life of volunteer EMS. Its getting much to busy. Instead of wining and carrying on about it, apply for the jobs, or focus on the patient's needs, not your own!

North Shore has some of the best medics in the metro area. Why shouldn't a patient call who they want. Besides most vollie ambulances force them to go to the closest hospital, which may not be necessarily good for the patient. Patient's like to go where their doctors are, or where the specialty tx is available. Not every EMS call is critical. People should have the choice of the best service available, and if they want to call someone else, so be it. Stop crying as if they're stealing your calls. You vollies make me sick. All you care about are yourselves, and your hobby. Got news for you, your days are numbered.

you complain that a "911" ambulance will only go to the closest hospital. Lets see, this is the duty crew now, they get a call. Closest hospital is 5-7 minutes away. The pt wants to go to lets say Stony Brook. You are now talking about 40 min ride ONEWAY which now means your district has now immediate 911 response because it is almost an hour away. You have now taken the duty ambulance out of service for that community for about 2 hrs. I dont think I would want that. If you want to go to a distant hospital then call a private ambulance. If you are picking and choosing where you want to go you dont really need an ambulance. Get someone in the family (usually there are 2 or 3 cars parked in the driveway), taxi or a private to take you there and leave the duty ambulance and crew in district to do what they are there to do, assist the community.
Besides most vollie ambulances force them to go to the closest hospital.
Are you REALLY professional ?
If you were you would know that the VOLUNTEERS do not FORCE patients to go to the closest ER, THE STATE DOES!
Doesn't your protocols state you must transport to the closest hospital ?
It's not a CHOICE its NYS protocol.

Interfacility transports (not 911) do not have to go to the closest hospital. There are lots of these companies (NS, LifeStar, AMR) that routinely operate throughout Suffolk and Nassau counties. And yes, they do not need permission from the local FD baron to operate in their kingdom.
A lot of these transports go to specialized care facilities like Cath Labs that are not necessarily available at the closest hospital. They often use specialized equipment like vents, balloon pumps, and neonatal equipment not available on 911 ambulances.
There is NO WAY vollie deparments are capable of providing this service.
NOT talking about interfacility transports, were talking about 911 calls, MEDICAL EMERGENCIES go to the closest hospital. And if its NOT a medical EMERGENCY people have NO business calling 911 !

Not what I am talking about...If you really think you know Article 30 then you would know that a patient CAN choose a hospital. In fact an EMT could be in violation if it were found that he/she took the patient somewhere where the patient was coerced. And if you think that doesnt happen, then you are fool.
I am talking about non-emergent cases that still generate a call to 911. Once you are on scene, you have a duty to act. Most will not transport out of the general area of their district. Thus, forcing the patient to go to St Catherine's instead of Stony Brook.

That doesn't always mean closest hospital.
You have
Burn centers, Stroke centers, Cath labs, Trauma centers (ya know the main reason for the SCPD hellicopter), Teritaty care centers, Hyperbaric chambers, OB (or do you always take a pregnant patient to a hospital without a nursery, peds or OB facilities).
Get the point.

Funny shit. When will some of these depts learn what hospitals can cover what services. Only a few will do peds, some will take OB and have newborn NICU facilities, and only SBUH in suffolk can do interventional caths , so all those chest pain pts that are brought to mather, st charles, st catherines and BMH go to either SBUH or North shore/LIJ when they need a cath with interventions. Stop wasting the pts time and bring them to a hospital that can help them, not just dumping them at the nearest place

So according to your logic, it would be better for the person with severe chest pain to go an extra 10 minutes to SBUH than to go to Mather and get stabalized in their ER, then get transported to SBUH (Isn't that what the privates are for anyway?)

Actaully going to the so called chest pain ER and being treared with thrombolytics that are only proven to work between 45 and 50 % of the time is not as good as taking the patients to a hospital with interventional cardiology services that are proven to work 98 to 99.9 % of the time.
And yes thats what the privates are for. Fixing the indiscretions of the people who think they are doing the right thing and not taking patients where they need to be.

They are being transferred because they can't be stabalized. If their cardiac episodes become stable then they do not need any further interventions. Get It! :?:

As was stated, Art. 30 says "nearest appropriate facility. This not only gives you permission to go to a further hospital, but in some situations, it means that you are acting negligently by bringing the patient to the nearest hospital. A good example of this is the new stroke protocol. As of right now, St. Charles and Stony Brook are the only two approved stroke centers in Suffolk County. The protocol says that if a patient is exhibiting signs or symptoms of an acute stroke, and the time from onset to arrival at the stroke center's door would be less than 2 hours, you must transport them to the stroke center. (Unless they are in extremis). That means that if you are in Patchogue, and your patient meets the criteria, you should be taking them to Stony Brook or St. Charles, not BMH. Or a trauma patient in Smithtown that should be going to SBUH, not St. Catherines. Think about what resources your patient needs, where they will get them, and do the right thing. At least pretend that we are professionals.

A Queens psych patient went berserk in the back of an ambulance on Sunday and assaulted two EMS workers - then fled in a stolen car and led police on a wild chase into the Bronx, police said.
Kenneth Ashley, 30, of Jamaica, was being transferred between Long Island hospitals when he attacked the first Emergency Medical Service worker, police said. The driver pulled over on Northern State Parkway about 2:15 p.m. and opened the back door of the ambulance to help his screaming partner.
Ashley chased both EMS workers onto the parkway, hitting one in the face repeatedly with a laptop computer, cops said. He also punched a good Samaritan who tried to break up the melee, then threw him over a guardrail.
Several off-duty NYPD cops were among the motorists who intervened. Ashley raced off in one of the cops' vehicles, police said, with cops pursuing him across Queens. He was arrested about 45 minutes later after he crashed on Bruckner Blvd. He was taken to Jacobi Medical Center for observation and charged with assault, grand larceny and reckless endangerment, cops said.

North Shore should go to the town and ask why the ambulance contracts are not posted so he can bid on them and dump the useless Islip ambulances. I am sure Islip Town is breaking some type of laws not posting ambulance contracts for outside agencies to bid on
