St. James has posted a job opening for an Advanced Life Support Provider.
I hope you studied your BSI procedures during your training because one of the main duties is cleaning up every possible bodily substance excreted by the volunteers.
You can see where their priorities are.

I am not sure you got Janitor Medic's point. I think he is pointing out the lack of respect this department has for ems as a whole. If they really cared they would ask for the highest certification like paramedic. Maybe require things like ACLS, PHTLS, PALS, National Registry etc. or how about ALS experience.... maybe some of that would rub off on their members. But no.... they want people who can push a broom.
There are plenty of jobs for qualified ALS providers. Tell these people to get a clue.
It's a bit of a catch-22 for St. James, if they know what that means, because only the worst medics would apply to work somewhere that requires janitor duties. Do they want a janitor for the girls at the house or do they want a decent medic?
Never any shame in having a job. Any "EMS professional" that would take this type of job should be completely and utterly ashamed of themselves. They should know fully that they are worthless to EMS, they drag everyone else down with them and should really just become a full-time janitor. They would garner far more respect from me by being a janitor rather than showing a total lack of regard for my profession. All fire departments that hire janitor/medics are really just looking for a maid and all janitor/medics that pick up after volunteers are pathetic bitches.

I know of one district that recently fired the house person and slapped the janitor duties on the EMS providers. There was no question this was going to happen, no respect for the profession even offered, basically it was here's what we're doing take it or leave it. nice. I cannot fathom how BOC's can believe that the person that they will trust their loved ones to is the same person that is handling clorox, floor stripper and all the other crap that is left about in a fire house after the vollies get done trashing the place after a function

any fat, beer swilling, ignorant, vollie fireman can do the floors.Leave the EMS function to the professionals..hire someone who passed a physical, backround and psych test, and has the credentials to do the job. Once again, the vollie service bastardizes its function, proliferating the insanity of the vollie service, with a few lackeys on the payroll...flush the vollie service down the tube where it belongs, and lets go paid entirely!!!!

No, the problem is that the volunteers take advantage of the situation. They figure that because the medic/janitor is there, they don't have to pick up after themselves, "after all, they're getting paid to do this stuff." If a volunteer CC does a job and tosses the used medication vials on the floor of the bus, who picks them up, usually the paid guy the next day when he/she is checking the truck. Does it always happen this way? Of course not, but it does make the volly less likely to put in the extra effort to keep the place clean when they know someone else is going to come through and pick up after them. The fire departments here on LI are wealthy enough to hire a real maid who will do a good job and clean everything.

In schools Teachers should be able to clean up their own classrooms and hallways.
Police officers should take the time out to clean the stations as well.

You all should go to St. James FD and see how the guys there are with the paid EMS guys, you don't see a mess left around after a bunch of guys have been sitting there, they get up and throw out their own stuff. And the EMS guys and the vollies are pretty damn friendly with each other, hanging out, laughing, having fun. You should all sit back and watch instead of being old wash women and talking about shit that don't go on. Actually, you are WORSE then old wash women. You're worse that women, period!

Cleaning is not beneath us. We all clean our own houses, and if I needed work, and all that I could find was a job cleaning Wendy's at night, I, like most of us, would do what I had to do for my family. That being said, it is entirely inappropriate for a medical professional of any level or certification to be performing cleaning duties in line with there medical duties. Anyone who knows me has heard me use the following example many times: It's 10am. Your are activated to respond to the local elementary school for a child respiratory. Being the diligent employee that you are, you were sweeping the floors in the firehouse when you hear the alarm. You cast aside your broom and race to the truck to respond. Upon arrival, you find said child in the nurses office experienceing exacerbation of their previously diagnosed asthma. Everyones' fears disappear as you immediately begin your assessment and appropriate ALS interventions. The child is rapidly packaged, and transport commences to the local ER. But wait! The child's mother, who came for the ride to the ER, is shocked and amazed to see that not only is her child not getting better, but in fact he is getting worse. When she asks why this is happening, you politely reply that your uniform is covered in dust and dirt from sweeping, and you are in fact triggering the worsening attack. Sorry, mom, but that's my job, too. This may be an exagerated example, but not completely improbable. It's about serving your community better, not laziness or "the job's beneath me". There's no problem having EMT's as housemen, as long as they are not also interacting with patients. As EMS providers, we are responsible for providing a clean, safe environment for our patients, and by having your EMT's and Medics double as housemen, you are compromising these ideals. If you feel that your call volume is too low to justify having EMS providers do nothing but EMS, assign more fitting roles, such as QA/QI, inventory management and ordering, drills and training, etc.

Maybe if you pathetic CC's would go to medic school then you could get a decent job at a reputable hospital. but because most true ems systems do not recognize (and rightfully so thank god ) CC level you continue to lower the standard. By being desperate to work as an ALS provider you take any job no matter what they ask you to do. instead of fixating on working as an ALS provider why don't you take a legit EMT job. And stop dragging down the salaries of professional paramedics that just want an easy second job. Most of the Jobs in this county were not meant for some half ass vollie to twist into a career.
SO go to medic school like the rest of us or go fuck yourself and stop complaining. Your lucky this backward County even allows you to practice at all. PS..I am the gayest medic in Suffolk

Pathetic CC's ??? Have you take a look at the protocols lately you god wannabe? There is very little difference between CC's and god wannabe's. The main difference is that we didn't get the "How to become a god 101 class" oh, and we didn't get sucked into pay 7k for the same classes. Sucker.... :lol:

Look i think were getting away with the scope of things here, so what your saying is that you want a paid ems position. and when there are no calls or nothing to check out on the bus, you should just sit around and wait for the alarm phone to ring?? I`m sorry but you can preach to the choir all you want to. I disagree. If i`m not mistaken by civil service account (and i may be wrong i admit this) but aren`t you classified as p/t firehouse maintainers? Ok then the other alternative is use you skills somewhere else. try to get on north shore ems or go work for a hospital in the city. you don`t have to clean there................... A job is a job if they pay well what`s the problem. I understand you went to cc or p school that`s great but like i said earlier nothing should be beneath you...................

fisrt of all if you think medic's and cc;s are equal in the feild that just goes to show you how little you really know and you ar just proving the other comments about cc's true.you don;t even have 1/3 of the field experience or text knowledge that we have...with that being said,hire a houseman.and yes it is beneath me.. if you want a janitor" fuckin hire one".and like said before the reason they do this is because people like you take these jobs and bitch up to doing jobs out of your scope...aka. cleaning,mopping mowing etc..we are paid {we being cc's and medics}for what we can do not for what you want us to do!!!!!

I don't know how things work over in the 4th, but I can tell you from personal experience the every single department in the 5th Division, with the exception of one, has paid EMS providers. And I can only think of a handfull (like 4 or 5 out of about 30 departments) that still require their EMS staff to perform custodial duties. And no, our Civil Service titles are for the most part not Fire House Maintainer (that's the list housemen get hired off), it's either EMT, EMT-CC, or EMT-Paramedic. There are specific titles for the positions for this exact reason. And as I said in my previous post, no one says that they have to just "sit around". FIND MORE APPROPRIATE EXTRA DUTIES FOR THEM. One of the districts that I work for had considered adding cleaning duties to our positions (we have a very low call volume, maybe one or two calls per shift), and after long discussion with the board where we voiced our objections, and they listened to us, the board agreed with us. We do perform additional duties, such as stocking, preparing orders, answering the phones, assisting the Rescue Officers, etc. BUT WE DO NOT CLEAN.
Additionally, as an EMT-CC who has just completed Paramedic School, I can tell you first hand that there is a world of difference between the two skill levels. It's not about the skills that you can perform, it's knowing why you're doing them. (And a cricothyrotomy is not used when you can't tube, it's for an airway obstruction that cannot be corrected by other methods. If you can't tube, you just bag). A CC does not equal a medic, at least not at first. Are there a few CC's who may be the equal of most medics, sure. But not the majority.

a cc never equals a medic. skills are skills anyone can learn them. the fact that you cc's keep using skills as an answer to this argument just shows that you really dont understand.
it is not about the skills it is learning the medicine behind the illness that is where you will never equal a medic.
there is a reason that most legit als employers only hire medics
ie: in the city, north shore, and even your own self appointed temple out here stonybrook.
that in itself should tell you about where you stand in the medical community.

these morons are just egotistical assholes and are the main cause of problems in the ems community. you should come down off your high horse and see really what matters here is patient care. yes there is a difference between a cc and a medic, but you are not god and should not act like it. If cc were as bad as you portray them to be, NYS would have abolished the position by now. So any medic that has something to say about the cc level should put a sock in it now because these rights to practice als care have been earned and granted to us by NYS DOH. You are not god so shut the fuck up assholes.

I'm a CC and I have worked with medics so allow me to tell you..if people think that all that seperates a CC from a medic is one skill you're insane. Many of the medics I have worked with teach CC's that there is a reason that you push the "pink drug" instead of the "purple one". I personally believe that I am not a paramedic, and do not sell myself as one to people. when asked what the difference between a CC and a medic is I tell people 1 skill and about 1000 hours of clinical and classroom education. though I have followed the guidance my Paramedic peers and have learned as much as can, I can't afford the 7 grand to be one of them but figure I can make a buck doing the job as a CC until I finally pack it in in the EMS field and move on to either an RN or PA level. As for sweeping and mopping, it's your station keep it clean.

But as you said in you protocol quote, If ETI is unsuccessful and airway remains obstructed, perform needle cricothyrotomy and ventilate with jet insuflator Do you see the part that says if the airway remains obstructed? This option is aimed at a resp. arrest secondary to airway obstruction. The idea is if you have tried everything else to clear the obstruction, inluding trying to push the obstruction through to the lungs with the ET tube (the surgeons can always take it out later) and the airway is still blocked, you do the crich to bypass the obstruction and make a new airway. if you're going to correct me, please try to be right.
